Your child’s name and date of birth
This must match the name on their birth certificate or passport.
Make sure your child’s primary school holds the same details on record. If these do not match your application may be considered invalid.

Applying for medical priority at a community primary school
All primary schools are required to make reasonable adjustments to accommodate children with a range of medical needs. However, in few exceptional cases there may be reasons why a child needs to attend a specific school and this could be due to the child or parent's medical condition. Council tax account number
If you are responsible for paying council tax at your child’s address you will need to enter your account number. This can be found in the top right corner of your Council Tax bill.
Supplementary information form (SIF)
Some schools may ask for more information to assess your application, which you must provide through a SIF. You can find out if this is required and download the form from the primary schools directory.
You will still need to complete an online application as well as a SIF.
