Information you need to provide

You should be able to tell us:

  • the date and place of the stillbirth
  • if the baby was a boy or girl
  • the baby's last name
  • the baby’s first name, if you want to give one
  • an email address for your funeral director if you are having a funeral - we need this so we can send them the green certificate for burial or cremation

We need these details about the baby’s mother:

  • full name, including previous last names if she has married
  • date and place of birth
  • occupation
  • address
  • date of marriage, if married to, or in a civil partnership with, the baby’s father at the time of the stillbirth

We will also ask if the mother has any other children. This optional information is only for statistical use and it will not be included on the register or any certificates.

If you are registering the name of the father on the register, we need to know his:

  • first and last names
  • date and place of birth
  • occupation
  • address

Medical certificate of cause of death (MCCD)

We need the medical certificate of cause of death (MCCD) for the registration.

Email the Register Office to check we have the MCCD before you book your registration appointment.

If the stillbirth occurred in Croydon University Hospital

The pregnancy loss specialist midwife will liaise with the bereavement team and send the medical certificate of cause of death (MCCD) to us.

If the stillbirth occurred in the community, at home or in a care home

The certifying doctor should send the MCCD to the Croydon Register Office.
