Who can get support
You might be able to get support if you:
- struggle with health or personal care
- provide unpaid care for someone
- have a disability or mobility issues
- have a hearing or visual impairment that affects your daily life
How we assess your needs
Your assessment can be done by phone or in person. You can also have someone with you.
We'll talk to you about the support you need, and we'll ask you about your daily life to find out how we can help.
Paying for care
You might have to pay for some or all of your care. You or someone else will need to complete a financial assessment to work out how much you need to pay.

If you're eligible, we'll work out what support suits you best. This might include:
- support from local services
- equipment and modifications to your home
- help from friends and family
