Serious case reviews

Serious case reviews (SCRs) are undertaken by local safeguarding children boards (LSCBs) for every case where abuse or neglect is known or suspected. If a case raises to concerns about the way in which local professionals and services worked together to safeguard and promote the welfare of children, then SCR is undertaken.

This can happen when:

  • a child dies, or
  • a child has been seriously harmed
