How to request for an assessment

All requests require the consent of the young person or family, unless the young people does not have the mental capacity to do it themselves.

Who can make a request

A request can be made by a:

• parent or carer
• person acting on behalf of the early year’s setting, school or post 16 college, this is usually the Special Needs Coordinator (SENCO)
• doctor
• family friends
• a young person – if they are aged 15 to 25
