What to do if someone in your business has an accident and how to report it.
All accidents, incidents and near misses, however minor, must be recorded by the business premises at which it occurs either in the accident book or electronically.
All deaths, major injuries and dangerous occurrences must be reported immediately to the Incident Contact Centre (ICC).

All other reportable injuries and diseases must be reported to the ICC within 10 days.

For all accidents please complete form F2508 within 10 days of the accident occurring.

Any accident arising out of or in connection with a commercial workplace in which an employee, or a self-employed person working on your premises is killed or suffers a major injury (including as a result of physical violence). Or a member of the public is killed or taken to hospital straight from your premises.
If there is an accident connected with your workplace (including an act of physical violence) and your employee, or a self-employed person working on your premises, suffers an injury which requires them to take over 7 days off work as a result.
When a doctor notifies you that your employee is suffering from a reportable work-related disease.
Any incident or near miss which occurs at your workplace which does not result in a reportable injury, but which may have resulted in serious injury or fatality.
