What happens in the assessment

The assessment may take place by phone or in person, it can be at your home or a location suitable for you. You can ask a friend or relative to be present to support you.

We will begin the conversation by getting to know you and your situation. We want to know about your typical day or week, what is important to you and what would help make a difference to you.

You can tell us about yourself, family, or friends. We would like to know about your hobbies, any health concerns, about your housing and money. Tell us if some things are difficult and how you think things can be made easier?
