Croydon's wheelchair service aims to meet the needs of individuals with a permanent need for a wheelchair, with the overall aim of achieving independence in daily living.
If you have a wheelchair to support your independence you, your next of kin, a care home or GP can contact us on their behalf and ask for a further assessment if your needs have changed significantly.
If you are not already working with the wheelchair service and you meet the eligibility criteria, a referral can be made by your GP, community nurse or allied health or social care professional. To make a referral, download our referral form (DOCX, 215KB), complete it and send to us, or email your request to the wheelchair service on cesadmin@croydon.gov.uk.
