Eligibility

First time mothers aged 19 and under who are residents in Croydon.

Referrals

Referrals can be made by any professional.  Clients are not able to self-refer but should talk to their midwife. If you are not sure who your midwife is, ask your GP, school nurse or teacher / college tutor.

What the service does

The Family Nurse Partnership is a voluntary home visiting programme for first time young mums, aged 19 or under (and dads). A specially trained family nurse visits the young mum regularly, from early in pregnancy until the child is two. Young women who are 19 and under and less than 28 weeks pregnant with their first baby, may be able to have a family nurse. Visits are provided in the home but can also be at the young person’s school or children’s centre if this meets the client’s needs.

What the service does

Health visitors provide a community-based public health nursing service which delivers health promotion in line with the Healthy Child Programme, and targets health inequalities to improve the health outcomes for children and parents. Babies are seen with follow up as appropriate, either at home or in a clinic setting. Routine developmental reviews of children aged 1 year and 2 to 2.5 years are also delivered by the service. Health visitors work in close partnership with families and other agencies to ensure all children are safeguarded. Targeted services are available to those with identified additional needs; for example enuresis service for over 6 year olds or those with special needs or vulnerabilities.

Visits are carried out in the family home for new birth visits. The service is also located in Croydon’s children’s centres and NHS premises.
