Many adults need support in their daily lives for different reasons. For some people, getting older means you can’t do as much as before; for others, you need help to cope because of a learning or physical disability, mental health needs or long-term illness.

Most people can manage with the help of family and friends. Our website has information, advice and guidance on the different services available that you can access directly.

Some people qualify for council-funded support at home to help them to live independently. For others, more intensive help may be needed and other options can be looked at, such as supported housing and residential or nursing care. The kind of support people need varies. You can find out how to access this support below.

You can apply online for a care and support assessment

Eligibility

To be eligible for social care support from us you must have needs caused by or related to a physical or mental illness or impairment, and leaves you unable to:

  • cook for and feed yourself
  • keep yourself and or your home clean
  • use the toilet independently
  • dress yourself
  • be safe at home
  • maintain healthy relationships
  • access and engage in work, training, education or volunteering
  • get out and about locally
  • carry out a caring role for a child

In addition to being unable to achieve two or more of the above, there must also be a significant impact on your wellbeing or quality of life. Wellbeing is defined as:

  • personal dignity (including treating the individual with respect)
  • physical and mental health, and emotional well-being
  • protection from abuse and neglect
  • control by the individual over day-to-day life
  • participation in work, education, training or recreation
  • social and economic wellbeing
  • domestic, family and personal relationships
  • suitability of living accommodation
  • the individual's contribution to society

Assessment process

During an assessment, we have a discussion with you that helps us identify your needs so the right support can be given.

As part of your assessment process we may also ask you to complete a full financial assessment to see if you are eligible for financial help from the Council.

Carers are also entitled to a separate assessment of their needs.

Support Planning

Following assessment we will work with you to develop a care and support plan. A support plan builds on information from your care needs assessment, focuses on broader life outcomes offering you choice and control in planning your care.

The support plan can help you to get the things you need to be:

  • more independent
  • healthier
  • safer

Developing your plan

We can help you to create a plan that reflects you as an individual, what is important to and for you and how your needs will be met. It will include:

  • your eligible needs
  • what support you receive - paid for and support provided by relatives or friends how much that support will cost – how you will spend your direct payment
  • the outcomes you are looking to achieve in your day-to-day life.

It will also include information about how you can remain as independent, active and healthy as possible.

What happens next?

After completing your plan, you can start choosing your support. You will have started thinking about some of the details when developing your plan. If you have decided to take a direct payment, you or someone acting on your behalf will work with our Direct Payment Support Service to start arranging your care and support.
