The supported self assessment questionnaire (SSAQ) is where you tell us about the level of your support needs. Information from the SSAQ will help us to estimate how much money will be available in your indicative personal budget to meet your assessed needs.
The amount of the personal budget will depend on the level of your needs. When your support plan is agreed the final personal budget amount will be confirmed.
You can complete the SSAQ online using the link on the right, or you can download a copy of the form using the link at the bottome of the page.
