Short-term domiciliary care
Assessment
You must have an Assessment of Need before we can decide if you may have any of the services below. The assessment aims to decide how best we can help. It is an opportunity for you to tell us about your circumstances and discuss your care needs.
Intermediate care
This care is provided to either prevent you going into or to help you coming out of hospital. It usually lasts for six weeks or less and consists of
- intensive home care to help you get back on your feet
- gaining new skills or relearning new ones in one of our reablement centres
- some physical help to regain the use of, for instance, your legs or arms, through occupational therapy or physiotherapy
Interim care
This means moving temporarily out of hospital into a Nursing or Residential Home while you decide your future, or sometimes while you have minor repairs or heavy cleaning carried out in your home. It can last up to six weeks and is a better option than staying in a hospital bed.
