Help leaving hospital

We work to return people back to their own homes and arrange domiciliary care for up to six weeks to help you settle back into your own routine.

As the six-week period comes to an end, we will contact you to remind you about this and see whether there is a need for any more care.

Cost

You may need nursing care once you are out of hospital. In that case you may need continuing care at Level 1 which is funded by Croydon Primary Care Trust. At Level 2 or 3 you will be given a contribution to your nursing care and we will assess whether you need to make a contribution to the cost of the care.

If you are assessed as being able to afford your own care, the Mayday Care Management Team, or the START Team if you are in a hospital outside of the borough, will help you with advice and provide lists of accredited homes. They will also move you into interim care or provide short-term reablement to assist you in moving out of hospital as quickly as possible once you are well enough and your condition is stable.
