Support Needs Assessment and Placement service – SNAP
The Support Needs Assessment and Placement service (SNAP) is a central assessment and placement service for vulnerable young people and adults who would benefit from supported housing.
The main aim of supported housing is to develop and sustain a person's ability to live as independently as possible either in their own home or in accommodation with staff on site.
People might need supported housing for a variety of reasons. They may have a mental health problem, a learning or physical disability, be homeless or have a history of offending. They may be a young person who hasn't had the chance to develop independent living skills.
Access to most supported housing services in Croydon, including visiting support services, is via the SNAP team. There are some exceptions such as Sheltered Housing for Older People, specialist drug and alcohol services and services for care leavers. The aim of the SNAP team is to ensure that people who most need - and would most benefit from - supported accommodation or visiting support in their own homes are given a suitable placement with one of the supported housing providers funded by the council.

When is the team available?
The team is available via the duty number from 9.30am to 5.00pm Monday to Friday (Wednesday 12.30pm to 5pm).
How does the SNAP service work?
All statutory and voluntary agencies can make a referral to the team.
Customers can also approach the team directly by coming to Housing reception at Taberner House during the times stated above. The Duty SNAP Coordinator will decide whether or not supported housing is an appropriate form of support for the person and if the SNAP team will carry out an assessment of the person's needs. They will match the applicant with a suitable supported housing service and make a referral to the service provider. There may not always be a vacancy to meet the needs of the vulnerable person, in which case the person would be placed on a waiting list until a suitable placement became available.
If supported housing is not the most appropriate form of support, the SNAP team will work with colleagues across the council to try to find appropriate alternatives.
