Palliative care
Croydon Primary Care Trust (PCT) commissions hospice and palliative care.
People living in Croydon with a Croydon GP are put forward for NHS continuing care services if they have a life expectancy of less than three months and have palliative care needs.
If you wish to remain at home, your care package is assessed by the relevant district nurse who has lead responsibility for providing the care.
Additional care is accessed by the continuing care team, including carers support services. A range of services can be provided including homecare, sitting services and nightsits.
The Crossroads Palliative Care Scheme also offers carer support for terminally ill patients and their carers. Alternatively, a nursing home placement or hospice bed may be more appropriate and can be accessed via the continuing care team.
Croydon is piloting the Liverpool Care Pathway scheme which is aimed at improving end of life care for patients and their families.
This is being co-ordinated by St Christophers Hospice in conjunction with two nursing homes to ensure that the nursing home staff learn from the skills of the specialist palliative care nurses and provide the best possible care for end of life in the nursing home setting.
Gold Standards Framework (GSF)
Used in primary care in the last six months of life. It involves identifying all patients at the end of life, keeping an up to date register, multi-professional practice meetings to assess needs and proactive plans of care.
Preferred place of care (PPC)
This is a patient-held record that helps patients and carers discuss and agree the care they would like at the end of life.
