Better care, higher standards - a charter for long term care
At some time in their lives, many older people and people with a long-term illness or disability may need help from their local housing, health or social services. Co-ordinated help from these services can make it much easier for people to get the support and advice they need to help them live independently and for their family or friends to support them.
A national charter, which you can download from the Department of Health website, tells anyone who needs care or support over the long term that local housing, health and social services should have a "local charter" that lays out standards for the services they provide and what to do if these expectations are not met.
Every local authority should have a "local charter" and we have provided this in pdf (portable document format).
This document brings together information about how to access health, housing and social care services and the standards users and carers can expect from services as well as showing how the Croydon Primary Care Trust and our Housing and Adult Social Services Departments will work together to make sure the most vulnerable people are given the care they need.
The document is set out under six main headings:
- helping users and carers to find out about services;
- understanding and responding to the needs of users and carers;
- finding a suitable place to live;
- helping people to stay independent;
- getting the right health care; and
- helping carers to care.
