Having your say at your review

A review is where changes can be made to your Care Plan. It is important that you give your views. Use this form to help you decide what you want to say. Ask an adult to help you fill it in if you want. Try and tick a box for each question and write any comments you wish to make.

You can open the form and print it so you can fill it out by hand or you can save it to your computer and type it up, then print it to give to your Independent Reviewing Officer.  There are three forms, so make sure you pick the right one for your age.

Consultation document for family members

These are to be completed by an appropriate family member prior to their child's Review Meeting. As a family member, it is very important that we hear your views on the plan for your looked after child, and the quality of the care you think they are receiving.

This form gives you a chance to tell us what subjects you would like to talk at the meeting. You can either bring it to the meeting or send it in advance to the Review Chair (address given at the bottom of the page).

You should be part of the meeting if at all possible, so that what is decided takes account of your views and wishes.

Don't worry about spelling or neat handwriting, and please ask your Social/Family Worker if you need any help completing it.

Consultation document for carers

For all LAC Reviews from 1 January 2007, there will be a new consultation paper for carers to complete. This should be completed for all looked after children and young people regardless of their age and placement type.

Carers should have them to complete between reviews, and should not receive them just before each meeting. If you are a carer, you can print off a copy from this web page or save it to your computer and fill it out electronically to either print or email to us.

This replaces the previous Department of Health (DoH) Foster Carers' Consultation Paper and all placement reports, providing a standardised and equitable service for all our looked after children and young people.

The completed document should be sent to the Independent Reviewing Officer (IRO) five days prior to the meeting in question, either directly or via the dedicated unit email address given below.
