Physical disabilities

Assessment

So that we know how we can help you, we will ask you what your difficulties are and how you think we can help. This is called an assessment of your needs and we will talk to you and your carers to work out how best we can assist you.

Service provision

Once an assessment of your needs has been carried out under Fair Access to Care Services (FACS), we can confirm what will be provided. If you meet the critical or substantial criteria we will provide those services. If you meet the moderate or low criteria we will direct you to an organisation that can help.

Eligibility

We can help if your primary condition is a:

  • physical disability
  • long-term illness
  • acquired brain injury.

If you have a physical disability but your primary disability is a learning disability, please refer to the learning disabilities page.

If you have been known to one of our mental health teams and have developed a physical disability, please refer to the mental health page.

If you have an alcohol or drug-related condition, please refer to the drug and alcohol services page.

If you have hearing and/or sight disabilities, you can get advice on sight disability services or hearing disability services.

Charges

Some of the services we provide are free and some we may charge you for. We will tell you how much you have to contribute to the service you receive.
