Assessment
So that we know how we can help you, we will ask you what your difficulties are and how you think we can help. This is called an assessment of your needs and we will talk to you and your carers to work out how best we can assist you.
Service provision
Once an assessment of your needs has been carried out under Fair Access to Care Services (FACS), we can confirm what will be provided. If you meet the critical or substantial criteria we will provide those services. If you meet the moderate or low criteria we will direct you to an organisation who can help.
Eligibility
We can help if your primary condition is a:
- physical disability
- long-term illness
- acquired brain injury
If you have a physical disability but your primary disability is a learning disability, please refer to learning disabilities.
If you have been known to one of our mental health teams and have developed a physical disability you need to refer back to them, please refer to mental health.
If you have an alcohol-related or a drug-related condition please refer to drug and alcohol services.
If you have hearing and/or sight disabilities you need the Sensory Impairment Team. It is important to refer to the appropriate team to receive the best service, please see sight or hearing disabilities.
Charges
Some of the services we provide are free and some we may charge you for. We will tell you how much you have to contribute to the service you receive.
