Self assessment for disability equipment
SARA - Self Assessment Rapid Access
SARA is an online self assessment questionnaire that asks a series of questions about your daily life and the difficulties you are having, and gives you tips and information on equipment and minor adaptations to your home.
We have developed the SARA questionnaire in partnership with the Disabled Living Foundation to include information on local services.
Where your situation is not complicated and the disability is not severe, the self assessment will offer you information and advice to help you independently manage your disability needs.
For example, if having a handrail by your front door would be helpful, or if you are having difficulty getting in or out of the bath, the assessment will generate tips and information on where you can see, try and buy suitable equipment.
Completing a self assessment
Most people find it takes about 30 minutes to complete the online self assessment. However, if you have lots of questions or are unfamiliar with computers it may take longer.
If you decide you want to try out or buy any of the equipment or minor adaptations, you can access the range through the Aztec Centre via the contact details below. You can also buy a small range of equipment direct from our online Specialist equipment shop via 'Do it online'. The centre is open on Mondays and Tuesdays from 10am - 4pm and it is best to make an appointment to avoid disappointment.
Alternatively, you can go to other local and national suppliers to see and buy a wider range of equipment or, if you feel you cannot afford to buy, it is possible to get some equipment on loan. (see the Specialist equipment suppliers 'see also' link).

When you have completed your assessment you will be given a report on your options.
And remember - completing the self assessment doesn't cost anything.
Need help completing the self assessment?
We have made arrangements with Age Concern Croydon (Croydon Disability Forum) to provide assistance for people who may have difficulty accessing the internet (including those for whom English is not the first language and for people with dyslexia) but still wish to complete the online SARA self assessment.

What the self assessment does not cover
This self assessment is not for council services such as meals on wheels, home care, day services or short breaks. More information on these services is available on this website.
Complex needs
If your needs are complicated, you may still wish to try the SARA self assessment first as it may have some immediate solutions for you. You can then contact Social Services for Adults to make a referral for a professional assessment. There is usually a waiting list and referrals are prioritised according to the level of personal need.
Your SARA report may be helpful to you, and you may feel better informed if you have a professional assessment.
